General Donation
General donations support the overall mission of the HHT Foundation, making possible all of our efforts towards education, support, advocacy, and research. It is thanks to our generous donors that we are able to offer our website, newsletter , 800 number, regional/national conferences, and other services, all while reaching out to physicians, health professionals, insurance officials, government officials and other professionals on behalf of those with HHT.

2008 HHT Membership Form
Research Donation
80 cents of every $1 donated to this fund is held in restriction and may not be used for purposes other than investing, thereby generating steady, ongoing income for the HHT Foundation. Annually, the Board of Directors allocates a portion of investment earnings from this fund to support research into the diagnosis, cause, management and cure of HHT. Research grant applications undergo a thorough peer-review process that includes an evaluation of scientific merit and how each project will advance knowledge about the fundamental processes of HHT and/or potential new treatments. Any donation may be made to honor, celebrate, remember or support a loved one. All tribute gifts that the Foundation receives are acknowledged by sending notification to the family or whoever you may specify of your kind gift.
A Memorial Gift - Often families who have suffered a recent loss ask that donations be made to the Foundation in lieu of flowers. Other times, individuals want to remember and celebrate a loved one’s life by making a special contribution in his or her name.
Gift to Honor Make a gift donation to honor someone special in your life such as friends or family members.
A Healthcare Professional Tribute Honor a healthcare professional especially helpful to you or some you love.
Special Occasion Gift Make a gift donation to the foundation to celebrate a special life events, such as weddings, anniversaries, birthdays, or special recognition by asking for donations to be made to the Foundation in lieu of gifts.

Workplace Giving
There are several ways to give to the HHT Foundation through, or in conjunction with, your workplace.
Matching Gifts
Many employers match their employees charitable donations via a Matching Gift Program. Check and see if your employer is one of them.
The process is simple. When sending in your donation to the HHT Foundation, include your company s Matching Gift Form. You will be asked to fill in a Part 1 (DONOR). Upon receipt, we fill in Part 2 (DONATION RECIPIENT), and return to your company. Soon thereafter, we will receive a matching donation from your employer. This is an easy way to double or even triple, depending on a company s matching policy your gift.
United Way
The HHT Foundation receives funds via many local United Way organizations. Does your workplace have a United Way campaign? Consider directing your United Way donation to the HHT Foundation. It s an easy way to support the Foundation s crucial mission of advocacy, education, and research.
Combined Federal Campaign (CFC)
The Combined Federal Campaign (CFC) is the federal government s version of United Way. All federal employees and military personnel can participate. To designate the HHT Foundation as your desired CFC charity, use our CFC Number: 10233.
Automated Giving
Many of our donors like the convenience of automated giving. In this giving method, you specify the donation amount and frequency whether it be monthly, quarterly, semi-annually, or annually. The charge is made to your credit card based on your specification and a receipt is generated and mailed to you with each transaction. Please contact our office and we will be happy to provide more information and/or set up an automated giving account for you.

Gifts of Stocks and Securities
Gifts in the form of stocks or securities are welcome! You can use the MEMBERSHIP/DONATION FORM to indicate what stocks you are planning to donate. The Foundation Office will contact you to facilitate transfer of your shares to the Foundation.

Planned Giving
If you are engaged in estate planning, please consider including a gift to the HHT Foundation as part of your legacy. Estate gifts can have a tremendous impact, enabling the Foundation to fund research and achieve goals otherwise unobtainable.

In-Kind Donations
The HHT Foundation is pleased to receive in-kind donations of anything from office equipment or supplies (that we can use in our daily operations) to airline miles or a stay at a vacation condo (that we can use in a raffle or auction fundraiser.)
The value of the donation for income tax purposes will be made according to IRS guidelines.
